Highlights

** ASDSM commentary does NOT replace a dentist's responsibility to read, know, understand, and implement the Standard of Practice.  Rather, the ASDSM's remarks are intended to underscore the regulatory standards **

1. Restricted Practice

According to the CDSA, dentists may only provide management of SDB (including snoring therapy) after completing a training program that is approved by CDSA for certification.  Of the courses acknowleded by CDSA, the ADSM recommends the University of Alberta OSA Certification program.

2. Diagnosis

Treatment of OSA must be proceeded by diagnosis.  Diagnosis of OSA is accomplished testing with level 1, 2, or 3 polysomnography, and interpretation by a sleep physician.  Under this standard Sleep Image does NOT qualify as diagnostic testing - even if interpreted by a sleep physician.

3. Records

The CDSA regulations clearly describe the minimum required records including: biometric data, patient history, intra-oral findings, and radiographic assessment.  A pre-treatment record of the diagnostic sleep test and a physician's recommendation for oral appliance therapy is essential for a dentists liability management, because it serves as a check-point to ensure co-morbid risks are properly managed.

4. Efficacy & Follow-up Testing

Once an MRD is titrated, efficacy testing is required to confirm effective disease management.  Furthermore, MRD efficacy ought be re-tested every 2 years or with a change in medical history - whichever happens first.

5. Multi-Disciplinary Patient Care

OSA is a medical disease that demands multi-disciplinary care for optimal outcomes.  Dental management of SDB demands medical collaboration - not only for direct sleep & airway concerns, but for proper management of co-morbid diagnoses (i.e. diabetes, hypertension, etc) who's management directly affects MRD effectiveness.
